Evelyn “Polly” Danielak (nee Boggs), of Island Lake, passed away Tuesday, August 19, 2014 at JourneyCare Hospice in Barrington at the age of 72.
One of thirteen children, she was born June 3, 1942 in London, Kentucky to Orville and Bessie Boggs.
In addition to her loving husband, Marty, Polly is survived by her children, Tammy and James (Deanna) Sizemore; grandchildren, Heather, Alexandria, and Richard James; sisters, Lois, Joyce, Mary, and Kathy; and brothers, Paul, Doug, Bill, and Richard.
She was preceded in death by her parents; brother, Ellis, Robert Charles, Lonas; and sister, Eulene.
A memorial visitation will be held Friday, August 29, from 4-9 p.m. at Davenport Family Funeral Home, 419 E Terra Cotta Ave., (IL Rt 176) Crystal Lake, Illinois 60014.
