Timothy William Puckett of Crystal Lake was born March 9, 1971 in Skokie and passed away unexpectedly Saturday, May 2, 2015.
Tim is survived by his wife of 20 years, Tracy; son, Ryan; sister, Julie (Roddy) Gordon; and parents, Kenneth and Laura Puckett.
Memorial visitation will be held from 11am until the time of the memorial service at 2pm, Saturday, May 9 at Davenport Family Funeral Home, 419 E. Terra Cotta Ave (Route 176), Crystal Lake.
